Full-Time Acute Dialysis Care
On-Site Home Hemodialysis (HHD) is a growing care model establishing partnerships between Skilled Nursing Facilities (SNF) and dialysis providers to offer in-house treatments to residents requiring dialysis. HHD significantly improves quality of life, and as such, residents see an increase in their functional ability and overall fulfillment. HHD reduces financial burdens for both patients and clinics, by eliminating time spent in clinics and diminishing the need for transportation.
